At St David’s Practice, we welcome any feedback good or constructive, we aim to give a high level of service with ease of use, in order to move with the ever changing climate within the NHS Primary care.
If you would like to share feedback with the practice:
1) Write to us! You can post this in the grey prescription request and feedback box next to reception at the surgery.
2) Use our online feedback forms (available on this website)
3) Submit a family and friends questionnaire
4) Respond to the SMS we send which asks for a review of your recent telephone consultation.
5) Email us!
If you have a need to complain, kindly write to or email us.
Once we have received a complaint in writing we aim to respond to confirm receipt within 10 days. We then have 28 days to investigate and take up in practice and management meetings, to then reply to the complaint in letter or email form depending on how it has been received.
We take all complaints seriously and all are taken up within Management and Partnership meetings.
